I hereby understand the monthly payment agreements made between myself and Break Free Worldwide LLC., which begin on today's date of registering, and end on the date of my last agreed upon payment. The agreed upon amount to be debited monthly from my account is understood, along with the monthly recurring payment date that I chose.

I/we realize that participation in Break Free classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the student and parent/guardian) assume all risks related to the use of any and all spaces used by Break Free. I/we agree to release and hold harmless Break Free including its teachers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Break Free liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Break Free. I understand that Break Free are licensed, accredited and insured organizations. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to any instructor or staff member as soon as possible.

I hereby grant permission to Break Free to use photographs and/or video of myself and/or my child taken during time at our facilities, recordings in our online classes, or at events related to our business, in publications, news releases, online, and in other communications related to the mission of Break Free.

You (the buyer) may cancel this agreement with 30-days written notice plus a termination fee of $147. You may use
your judgement and have the liberty to finish out the final 30 days of the membership. The cancellation notice must be signed off by a Break Free staff member with a copy made for your discretion. I (the buyer) acknowledge these terms,
and understand that unless I make the payment of these cancellation fees, my account will continue to be on file with
attempts to debit the amount owed on the agreed upon terms. If my account is returned as delinquent or incorrect for
2-months straight with no attempt of contact made to Break Free, than I understand my information will be turned
over to collections

I agree to have sufficient funds in my account provided in order to meet any recurring payment terms agreed upon, and if my account is to ever return as delinquent or incorrect, then a $25 late fee will be assessed on your account to recover any funds lost on Break Free's end for the unsuccessful attempt to charge your account.

Access to the student’s classroom, whether on-site or online, is granted within a 24-hour period pending the confirmation of the first payment posting. For any account that goes delinquent and occurs a balance owed, or the terms set for the initial option chosen come to an end, the student is removed from classroom rosters and is denied access until the balance owed has been paid, or the next option chosen for the student who has finished their initial terms has been confirmed and paid for.

Upon dissatisfaction of the program within the first 30 days of signing on for registration, the account is allowed a termination of the terms agreed upon with no cancellation fee or policy in affect. The registration fee and first month's payment taken upon signing is non-refundable.

Refund requests for any payments are requested via the Refund Request Form, and are pending valid reason and approval from the Break Free Accounting department. Any approved refunds are processed on two designated days, and are subject to 3-4 day processing time once issued.

For any recurring payments options, there is an allowance for a “hold” of a maximum of one-month payment due to reasons which place the student in circumstances where they are not able to attend class. This includes but is not limited to; injury, stress at home, and general hardship. A doctor’s note must be presented for any injury or health related matter. For any matter where an outside document cannot be presented to verify justification for the hold, the account holder is required to write a brief letter to Break Free stating the request to hold and the reason for it.
